Picture Permission Form

I, , hereby give Capri Plaza Pet Clinic, the absolute and irrevocable rights to use my pets name and pictures, and my name on the web site of Capri Plaza Pet Clinic for display, public relations and marketing. I hereby acknowledge receipt of adequate consideration and waive the right to charge for use of the pictures and my name or to inspect or approve the images prior to any form of usage. I understand that the images may be modified to be used as design elements.
